Fellowship Application Guidelines
To submit an application to the Ear Research Foundation Fellowship Program, qualified candidates must complete and return:
Send all of this information to the Ear Research Foundation, 1901 Floyd Street, Sarasota, FL 34239 or mlane@EarRF.org
Upon receipt of the completed application and review, candidates may be notified for an interview in Sarasota. These usually take place on Monday and Tuesday, and candidates should plan to spend at least two days on site.
The Fellowship is usually awarded a minimum of one year in advance, which allows time to obtain the necessary certifications.
A Florida medical license must be obtained and applications for surgical privileges must be made immediately to Sarasota Memorial Hospital, Cape Surgery Center, and Lakewood Ranch Medical Center. In addition, the Fellow must obtain certification to use the various lasers for surgery. The appointed Fellow must also sign a restrictive covenant.
If it is not possible for you to obtain the above-mentioned licensure, the Foundation does arrange unsalaried research Fellowships. If this is the case, please note your request to do so in your cover letter.

- A complete copy of your Curriculum Vitae
- Any reprints of your published material
- Three letters of recommendation (one must be from your Chief)
- A passport size photograph
